APPLICATION FOR INDIGENT HOUSEHOLD SUBSIDY : CHECK LIST
- Certified ID Copy of Applicant/s
- Proof of Total Household Income
- Affidavit if unemployed
- Electricity Token Receipt
- Declaration must be stamped with the Police Commissioner's stamp
- Proof of Residence
- Certified ID copies of Household Members
- Death certificate if household is child headed
